Certification Training and Ongoing training.  Are there operators not certified working the rig.
Floracain damage can be caused by winter cold
Spray rig  Calibration Spray rigs are notoriously out of conformance. 5 pct typical and in some cases over 20 pct.
How much actual chemical was purchased, how many ounces were  applied, how many ounces was left over on how many acres.
Should be able to show that the number of acres times 5 ozs  matches the volume consumed.
Spray rig travel rate can affect over-spray
Lack of Spray Shields
Lack of consistancy of tank mixes between growers.
Variances in amount of crop damage
Variances about field in amount of damage suggesting possible mixing issues with tank mix.
Incomplete Spray Records no listing of spreader sticker Kissan. The brand can make a difference.
Not Following Standard Practices Regarding Tank Mixes IE testing on plot new mixes. . Also jar tests.
Field Men are notoriously under informed due to data overload.
Tank Mix Order
Water Quality should be able to be shown as acceptable in ph and hardness and not contaminated with chemicals even though apparently satisfactory. 
Soil Chemistry and Type 
Residual Chemicals in Soil.  Chemical can build up over time.
Phytopthora Species... Root rot noted in Peachey study caused by tank mix.   Miller works in collaboration with Peachey.
Weather (seems to have been addressed)
 
Application was less than allowed but higher than minimum. :::  Post event Miller Suggests that farmers use less Callisto .

Qualtiy assurance program.  Being able to demonstrate that there is documented attention to quality is a part of best practices.
